Tattoo Request Form
For all bookings my scheduling app will hold a card for a $100 deposit if the appointment is missed with out 24 hours notice or communitcation. All smaller tattoos will be priced by the piece, depending on detail, size and placement starting with a $100 minimum. All large projects such as sleeves and backpieces will be at an hourly rate of $150 per hour.
